This question is from a member of our community that wishes to remain anonymous:
We have had trouble with the chair. He happily lets everyone else do the work and takes full credit, won’t help, and only uses his title if he doesn’t agree with a decision we have made whilst running the charity. 
We recently had a meeting and found he had been doing things without consulting the rest of us and also has a conflict of interest with many other organisations. We have had mediation with him to try sort some issues but feel they are not been dealt with, it came to head at a meeting and one of the trustees called a vote of no confidence against him.
He has since gone onto the charity comms website and removed another trustee as primary admin and made himself it, meaning we can’t log on.
