Do all trustees have to sign the same physical trustee declaration form?

I know it is good practice for trustees and senior managers to sign the Charity Commission Auto Disqualification Declaration annually.
But is it also necessary to sign the Charity trustee: declaration of eligibility and responsibility annually? Thanks in advance
